Requirements for Forklift License Plates

One: Inspection submission requires documentation.

1. Factory Internal Mobile Vehicle Inspection Forms (two copies)

2. Forklift Certificate of Compliance (original and copy each)

3. A copy of the unit code certificate.

4. A copy of the business license of the owning unit.

5.  Not handled directly by the legal entity (a power of attorney must be submitted)

Two: Application for driving permit requires documents

Special Equipment Registration Forms (two copies)

2. Inspection Certificate (original and one copy)

3. Forklift Certificate of Conformity (original and copy each)

4. A copy of the entity's unit code certificate.

5. A copy of the business license of the entity in possession

6.  Non-legal entity handling (a power of attorney must be submitted)

7. Forklift Management System and Catalogue each provided

8. Original and a copy of the driver's license (Note: Sealed with the employer's official stamp)
